Lincolnshire Social Prescribing

Social Prescribing is personalised support to meet the needs of an individual at a time when they need it most.

A social prescriber can help with a multitude or things such as :

  • accessing community groups and service,
  • dealing with anxiety,
  • depression and stress,
  • loneliness’ and befriending,
  • overcoming social isolation,
  • bereavement,
  • finance and housing support,
  • volunteering or returning to work

Please speak to any member of our team for a referral to the social prescribing service. Please note that patients must require support in one of the above areas and also consent to the referral being made and their health record information being shared with local NHS regulated services.
